Not always. That is by far the most common (and most stable) form of carbon, but forms with 8 (and, I think, 7) neutrons exist as well.
Besides, if you break it down further, you have 6 electrons, 18 up quarks, and 18 down quarks. (A neutron is one up, two down; a proton is two up, one down.)

In its most stable form, yes. Not all carbon is like that, though. Of course, protons and electrons are not the smallest particles you can find.
