First off, Obama is not the first president to have had mistakes during the recitation of the oath. In fact, there were four other presidents before him where the oath was flubbed.
Second, placing one's hand on a Bible is not a requirement for taking the oath. There were several presidents before Obama that did not use the Bible at all when taking their oaths.

The only picture released of Obama taking the second oath shows him with his right hand up and his left hand at his side. Apparently he didn't use any book at all for the second oath.
