The doctors still decide.

Wrong. HMO boards decide to refuse treatment based on what is "cost-efficient". With Public Healthcare what happens is:
1. You come in needing medical assistance.
2. Doctors decide how to treat you, money is not a consideration.
3. You get better.
4. Then doctors lodge a claim to get paid for all the costs of administering those treatments.
5. Then they get paid. The people rubber-stamping these claims are just hospital administrative staff.

Yes public healthcare costs money, but it pays for itself in how many more taxpaying workers will be able to return to work sooner and living. And if you're worried about federal deficits then:
1. Protest that America forms 49% of world military spending annually.
2. Protest Costly, pointless wars.
3. Protest bailing out weak, useless companies.
4. Protest how fucking much you spend on your leaders. A personal detail of hundreds? Two ultra-advanced private jets? Our Prime Minister takes normal commercial planes and is quite happy to do so!

The "extremely heavy fines" for not having private healthcare in Australia is a 1% extra levy on your tax for using the public system. That way the rich use private insurance and it costs them less. And 1% is only charged if you fall into a tax bracket (so those at the very bottom get it for free).

HMO's who decide the poor aren't worth saving and that pre-existing conditions (like genetic ones) aren't worth saving either.
